FormRelay Terms of Service

These App Terms govern your use of FormRelay, a Webflow App published by Faizatech. They supplement the general Faizatech Terms & Conditions and apply in addition to them. Where the two documents differ on a point that is specific to FormRelay, this page applies. By connecting a Webflow site to FormRelay, you agree to these App Terms.

Scope of these terms

These terms cover FormRelay only. They do not cover any other Faizatech product.

FormRelay was previously listed under the name Form Integrations. References to that older name mean the same App.

Faizatech also sells Webflow Designer applications that are activated with a license key. FormRelay is not one of those. It is a Data Client App with a per site subscription, no license key to enter, and nothing to activate inside the Webflow Designer. Where the general Faizatech terms describe license keys, that description does not apply to FormRelay.

The general terms are on the Terms & Conditions page. Data handling for FormRelay is described in the FormRelay Privacy Policy, which is the policy that applies to this App. The general Faizatech Privacy Policy covers the Faizatech website and the license key apps.

What FormRelay does

FormRelay is a Webflow Data Client App. After you authorize it on a Webflow site, it registers its own form submission webhook on that site. When a form on the site is submitted, Webflow sends the submission to FormRelay, and FormRelay delivers the fields to the one destination you mapped to that form. Payloads received from Webflow are verified with an HMAC signature before FormRelay accepts them.

FormRelay has no Designer Extension. It does not inject code into your site, and it does not use the Webflow Custom Code API.

FormRelay asks Webflow for these permissions: authorized_user:read, sites:read, sites:write and forms:read. It uses them to identify the authorizing user, read the site you selected, read the forms on it, and register and remove its own form submission webhook.

Each form delivers to exactly one destination. The destinations available today are Notion, Slack, Discord and Airtable (connected by signing in to the destination), HubSpot, Mailchimp, Klaviyo, Brevo, MailerLite, EmailOctopus, Moosend, Omnisend and Sender (connected with an API key), and Webhook, Zapier and Make (connected with a URL you supply).

What your subscription grants

Your subscription is per Webflow site. It is not a license key, and it is not tied to a seat, a user, or a number of activations.

Authorization is per site as well. Webflow asks you to pick a single site on its consent screen, and the access token FormRelay receives covers only the site you picked. To connect a second site, you authorize that site separately and subscribe it separately. Subscribing one site does not cover another.

A site delivers nothing until it is subscribed. Before that, FormRelay records that a submission arrived and does not store its contents. The same applies to a form that has no destination mapped: only the fact of the submission is recorded, not its values.

While your subscription is active you may connect the destinations listed above, map forms to them, view and resend delivery attempts, and export your delivery history.

Eligibility

To use FormRelay you need all of the following.

  • A Webflow account with permission to authorize an App on the site you want to connect.
  • A published Webflow site. Webflow sends form submissions from published sites, so an unpublished site produces nothing for FormRelay to deliver.
  • An account at each destination you connect, on a plan at that destination that allows the use you intend.
  • A FormRelay account, created with an email address and password or by signing in with Google. You are responsible for keeping access to that account secure.

Your data and your responsibilities

The form data you route through FormRelay is yours. Faizatech processes it on your instruction, to deliver it to the destination you chose and to record whether the delivery worked.

You are responsible for having the right to collect that data and to send it to the destination you pick, including any notice or consent your own visitors are owed and any restriction that applies to the kind of data you collect.

You are responsible for the destination credentials you give FormRelay. API keys, access tokens and destination URLs you save are encrypted at rest and are not shown again after saving. Use a credential with the narrowest permissions the destination allows, and revoke it at the destination if you stop using FormRelay.

For the Webhook, Zapier and Make destinations you nominate the endpoint yourself. FormRelay sends the submission to the URL you enter. It does not verify who controls that address and it has no visibility into what happens to the data once it arrives. Confirm the endpoint is one you control, and that it can receive the data, before you save it. You are responsible for the endpoint you nominate and for anything done with data delivered to it.

Data that reaches a destination is then handled under that provider's own terms and privacy policy, not under these terms.

Outbound deliveries leave from three static addresses: 208.77.244.240, 152.55.184.241 and 152.55.185.190. If your endpoint restricts inbound traffic, you can allow those addresses.

Acceptable use

You agree not to do any of the following.

  • Use FormRelay to collect passwords, one time codes, payment card numbers, or other credentials through a Webflow form. FormRelay drops fields whose names look like credentials before anything is stored or delivered, but that safeguard is not permission to collect such data. A Webflow form is not a suitable place for it.
  • Route data you are not allowed to hold, or send it to a destination you are not allowed to send it to.
  • Use FormRelay to send unsolicited messages, or to load a destination or a nominated endpoint with traffic it did not agree to receive.
  • Try to reach another customer's data or account, interfere with the service, or work around the limits set by Webflow or by a destination.
  • Resell, redistribute, or share access to FormRelay outside your own organisation, or reverse engineer the App, except where law allows.

Payments & billing

FormRelay costs 4.99 US dollars per site per month, or 49.99 US dollars per site per year. There is no free tier. The price applies to each connected site.

LemonSqueezy is the merchant of record for all payments. Checkout, tax handling, receipts and billing records come from LemonSqueezy.

Subscriptions renew automatically at the price and interval you chose, until you cancel. You can cancel at any time. Cancelling stops the next charge, and the site keeps access until the end of the period you have already paid for. Partial periods are not refunded.

To request a refund, email contact@faizatech.com within 14 days of the purchase.

If a price changes, the change applies from your next renewal. It does not change a period you have already paid for.

Destinations bill separately, on their own plans and at their own prices. FormRelay adds nothing on top of what a destination charges, and Faizatech has no control over destination pricing or plan limits.

What to expect from the service

FormRelay attempts each delivery. If an attempt fails, it tries again with a growing delay between tries, up to three tries in total, before it records the delivery as failed. Each delivery is recorded with its outcome, including the error message the destination returned. A failed delivery can be replayed from inside the App while its submission data is still stored.

Faizatech does not offer an uptime guarantee or a service level agreement for FormRelay. The service may be unavailable during maintenance, during an incident, or because of a problem at a provider Faizatech depends on.

Destinations are run by other companies. A destination can change its API, change its plan limits, rate limit a request, reject a submission for its own reasons, or stop working. Webflow can also change its APIs. Those changes sit outside Faizatech's control, and a delivery can fail because of them.

Suspension & termination

You can stop using FormRelay at any time. Removing a site from inside the App deletes the FormRelay webhook from that Webflow site, so Webflow stops sending submissions to FormRelay, and it also deletes that site's stored submissions, delivery records and saved destination credentials. Deleting your account does the same for each connected site. You can also remove the App's access from your Webflow account using Webflow's own App settings, which also stops deliveries.

Faizatech may suspend or end access to FormRelay if payment fails, if the App is used in a way these terms do not allow, if the use puts the service or another customer at risk, or if Webflow or a destination requires it. Where it is practical, we will contact you first and give you a chance to put the problem right.

What happens to your data

Stored submission payloads are removed 90 days after the delivery attempt. The delivery record itself, which destination it went to, when, and whether it worked, remains without the payload. Delivery records are deleted 395 days after the attempt.

You do not have to wait for those periods. From inside the App you can export your delivery history as CSV or JSON, delete stored payloads, delete individual delivery records, or delete your whole account.

If you delete your account, your account record, your connected sites, your delivery history including any stored submissions, and the destination credentials you saved are deleted, and the FormRelay webhook is removed from each connected site. Data that was already delivered to a destination stays at that destination. Only you can remove it there, using that provider's own tools.

Intellectual property

FormRelay, its source code, its interface, its documentation, and the FormRelay and Faizatech names and branding belong to Faizatech. These terms give you the right to use the App while your subscription is active. They do not transfer ownership of anything.

Your form data stays yours. Faizatech does not claim ownership of it and does not sell it.

Third party names & independence

Webflow, Notion, Slack, Discord, Airtable, HubSpot, Mailchimp, Klaviyo, Brevo, MailerLite, EmailOctopus, Moosend, Omnisend, Sender, Zapier, Make, LemonSqueezy, Railway, Resend and Google are trademarks of their respective owners. FormRelay uses those names only to identify which service a destination or a provider connects to.

FormRelay is an independent App published by Faizatech. It is not affiliated with, endorsed by, or sponsored by Webflow or by any of the destinations it connects to.

Disclaimer & limitation of liability

FormRelay is provided as it is, to the extent the law allows. Faizatech does not promise that every delivery will succeed, that the service will be available without interruption, or that a destination will keep behaving the way it behaves today. Implied warranties are disclaimed where that is permitted.

In plain language: use FormRelay as a way to move form submissions to a destination, not as the only copy of them. Webflow keeps its own record of form submissions, and you can export your FormRelay delivery history at any time.

Faizatech is not liable for indirect or consequential loss, for lost business or lost profits, for a submission that is delayed, lost or duplicated, or for what a destination or an endpoint you nominated does with data after it has been delivered. Where liability cannot be excluded by law, Faizatech's total liability for any claim relating to FormRelay is limited to the amount you paid for FormRelay in the 12 months before the claim.

Nothing in these terms limits liability that cannot be limited by law.

Changes to these terms

We may update these terms, for example when a destination is added or removed, or when the way billing works changes. The current version sits on this page with the date it was last updated. If a change affects what you pay or how you may use the App, we will contact subscribed customers by email before it takes effect. Continuing to use FormRelay after a change means you accept the updated terms.
